Following up on my last post, here is an excellent website to help you prepare for and create your Advanced Directive. The AD is a document you create where you designate a person or persons to make healthcare decisions for you (a healthcare proxy), and document your wishes and desires regarding what kind of treatments you want in given situations, what is important to you to help guide your proxy and healthcare providers in making decisions that are in alignment with your values. This website gives a lot of information and guidance in simple to understand videos and pages as it guides you through creating your Advanced Directive. I hope this is helpful.
