National Healthcare Decisions Day April 16, 2021

National Healthcare Decisions Day (NHDD) exists to inspire, educate and empower the public and providers about the importance of advance care planning. NHDD is an initiative to encourage patients to express their wishes regarding healthcare and for providers and facilities to respect those wishes, whatever they may be. ***


Conversations and planning about our wishes and desires around illness and end of life are difficult, but so important. Even if you are healthy today, circumstances can change in an instant. Completing your Advanced Care Directive and other guidance for your loved ones to follow if you are unable to make those decisions for yourself can bring peace of mind to both you and your family, knowing your needs and desires are clearly defined, and giving your loved ones a clear basis for making decisions for you. National Healthcare Decisions Day is a day to initiate this conversation and action.

For actually completing your Healthcare Directive, I recommend 𝗙𝗶𝘃𝗲 𝗪𝗶𝘀𝗵𝗲𝘀.𝗼𝗿𝗴. Here you will be able to fill out online, or order a booklet/form, to create your Advanced Directive.
